Recovery of villous architecture in gluten enteropathy exacerbated by gastro-jejunal anastomosis.

چکیده
Two patients are described whose clinical condition deteriorated following gastric surgery for peptic ulceration. Both were demonstrated to be suffering from gluten enteropathy with generalized malabsorption which included lactose. Following treatment with a gluten-free diet over a 2-year period the jejunal mucosa showed a marked improvement of villous architecture associated with a return of lactose absorption to normal in one patient. This supports the view that following a gastro-jejunal anastomosis the gastric secretion is not itself injurious to the jejunal mucosal structure.

Evidence of altered structural and secretory glycoconjugates in the jejunal mucosa of patients with gluten sensitive enteropathy and subtotal villous atrophy.
The pattern of lectin histochemistry in formalin fixed, paraffin embedded normal jejunal and subtotal villous atrophy specimens from patients with gluten sensitive enteropathy were compared. There was no significant difference in the binding pattern of five lectins (Arachis hypogaea, Canavalia ensiformis, Lens culinaris, Phaseolus vulgaris and Triticum vulgaris) between normal and abnormal spec...
متن کاملThe effect on jejunal mucosa of withdrawing and adding dietary gluten in cases of idiopathic
Biopsy studies of the jejunal mucosa in patients with idiopathic steatorrhoea after periods on a gluten-free diet showed that the epithelium improved quickly in respect of surface cell height, mucosal thickness, and mitosis count, and more slowly in respect of villous height and width. In no case was recovery complete in every particular. Appearances seen through the dissecting microscope impro...

متن کاملSeronegative Intestinal Villous Atrophy: A Diagnostic Challenge
Celiac disease is the most important cause of intestinal villous atrophy. Seronegative intestinal villous atrophy, including those that are nonresponsive to a gluten-free diet, is a diagnostic challenge. In these cases, before establishing the diagnosis of seronegative celiac disease, alternative etiologies of atrophic enteropathy should be considered. Recently, a new clinical entity responsibl...
متن کاملRefractory Celiac Disease
Refractory celiac disease (RCD) is when malabsorption symptoms and villous atrophy persist despite strict adherence to a gluten free diet (GFD) for more than 12 months and other causes of villous atrophy have been ruled out. RCD is considered a rare disease and almost exclusively occurs in adults. Persistent diarrhea, abdominal pain, weight loss are the most common symptoms in RCD. Also, anemi...
